ABSTRACT

Boletim que tem como objetivo demosntrar qual é o perfil epidemiológico dos casos humanos de brucelose no estado de Goiás, além de fazer o comparativo dos municípios com focos da doença em animais , haja vista que se trata de uma zoonose de ampla distribuição mundial e que gera impactos econômicos e sociais. Trata-se de um estudo desenvolvido baseado no banco de dados do Sistema de Agravos e Notificação (SINAN-NET), do Gerenciador de Ambiente Laboratorial (GAL), de dados fornecidos pela agência Goiana de Defesa Agropecuária (AGRODEFESA) e da Coordenação Estadual de Zoonoses da Gerência de Vigilância Epidemiológica de Doenças Transmissíveis GVEDT/SUVISA-SES-GO da Secretaria de Estado da Saúde de Goiás (SES/GO). É um estudo de caráter quantitativo, descritivo e retrospectivo entre janeiro de 2018 e abril de 2023, com dados analisados baseados em casos notificados e confirmados de brucelose humana por município de residência


Bulletin that aims to demonstrate the epidemiological profile of human cases of brucellosis in the state of Goiás, in addition to comparing municipalities with outbreaks of the disease in animals, given that it is a zoonosis with a wide worldwide distribution and that generates economic and social impacts. This is a study developed based on the database of the Diseases and Notification System (SINAN-NET), the Laboratory Environment Manager (GAL), data provided by the Goiana Agricultural Defense agency (AGRODEFESA) and the State Coordination of Zoonoses of the Epidemiological Surveillance of Communicable Diseases Management GVEDT/SUVISA-SES-GO of the State Department of Health of Goiás (SES/GO). It is a quantitative, descriptive and retrospective study between January 2018 and April 2023, with analyzed data based on reported and confirmed cases of human brucellosis by municipality of residence

Resumen La brucelosis es una zoonosis desatendida y un problema de salud pública en países en desarrollo, que también causa grandes pérdidas económicas en la ganadería. En México es endémica y Chiapas un estado al sur del país, destaca por la presencia de la enfermedad en humanos. Por medio de la revisión del estado del arte, se encontró que factores como la alta producción de ganado bovino, las limitadas estrategias institucionales de control de la enfermedad en el ganado, la falta de regulación sanitaria del género Brucella en productos lácteos y cárnicos y la ubicación del estado de Chiapas en la frontera con Centroamérica, podrían ser condicionantes para la prevalencia de brucelosis humana en esta región de México.


Abstract Brucellosis is a neglected zoonosis and a public health problem in developing countries that also causes great economic losses in livestock. In Mexico it is endemic and Chiapas, a state in the south of the country, stands out for the presence of the disease in humans. Through the review of the state of the art it was found that factors such as the high production of cattle, the limited institutional strategies to control the disease in livestock, the lack of sanitary regulation of the genus Brucella in dairy and meat products and the location of the state of Chiapas on the border with Central America could be determining factors for the prevalence of human brucellosis in this region of Mexico.

ABSTRACT

Brucella ovis, a non-zoonotic species, is the etiological agent of ovine brucellosis, an infectious disease of clinical or subclinical occurrence in sheep flocks. Until then, there is no serological study of anti-Brucella ovis antibodies in purebred sheep herds. This study aimed to determine the presence of anti-Brucella ovis antibodies in purebred sheep flocks with breeding purposes from Parana State. Blood samples from 728 animals, of which 563 were females and 165 males, between 8 and 56 months of age from the six major sheep producing mesoregions of Parana, were submitted to detection of anti-Brucella ovis antibodies by the Agar Gel Immunodiffusion technique using an antigen from the bacteria Brucella ovis (Reo 198). The results indicate the presence of this disease in purebred sheep from Parana State in a low occurrence of 0.27% (2/728). The only two positive animals were rams, Santa Inês breed, from the same flock in the East Center region of Parana, without clinical disease. In conclusion, Brucella ovis is present in purebred sheep in Parana State, Brazil, and this low occurrence may have occurred due to rigorous breeding systems that may contribute to reduce the transmission of this disease.(AU)


Brucella ovis, espécie não zoonótica, é o agente etiológico da brucelose ovina, doença infecciosa de ocorrência clínica ou subclínica. Atualmente, não existe estudo sorológico de anticorpos anti-Brucella ovis em rebanhos de ovinos puros de origem. Este estudo teve como objetivo determinar a presença de anticorpos anti-Brucella ovis em rebanhos ovinos de raça pura de origem, com fins reprodutivos do estado do Paraná. Amostras de sangue de 728 animais, sendo 563 fêmeas e 165 machos, entre oito e 56 meses de idade, pertencentes a seis principais mesorregiões produtoras de ovinos no Paraná, foram submetidas à detecção de anticorpos anti-Brucella ovis pela técnica de imunodifusão em ágar gel usando-se um antígeno da bactéria Brucella ovis (Reo 198). Os resultados indicam a presença da doença em ovinos puros de origem do estado do Paraná em baixa ocorrência de 0,27% (2/728). Os dois únicos animais positivos foram reprodutores da raça Santa Inês, do mesmo rebanho da região Centro Leste do Paraná, sem manifestação clínica. Em conclusão, Brucella ovis está presente em ovinos puros de origem no estado do Paraná, e essa baixa ocorrência pode ter ocorrido devido a sistemas rigorosos de criação, que podem contribuir para a redução da transmissão dessa doença.(AU)

INTRODUCCIÓN: La brucelosis humana es un problema zoo-sanitario global. México es uno de los principales países que se ven afectados, el diagnóstico oportuno y las pruebas serológicas confirmatorias son la base para la detección. OBJETIVO: Conocer la frecuencia de casos confirmados de brucelosis en diferentes Unidades de Medicina Familiar del Instituto Mexicano del Seguro Social en el estado de Puebla, México. PACIENTES Y MÉTODOS: Estudio transversal en pacientes de ambos sexos, adultos y pediátricos, con manifestaciones clínicas sugestivas de brucelosis; se realizaron pruebas serológicas para el diagnóstico confirmatorio. RESULTADOS: De un total de 77 pacientes, se obtuvieron 39 (50,6%) casos positivos, 21 (27,3%) casos saliendo de la infección, 9 (11,7%) negativos y 8 (10,4%) con memoria inmunológica; de los casos positivos, 32 (82,1%) eran adultos y 30 (76,9%) fueron del género femenino. CONCLUSIÓN: Del total de muestras, la mitad tuvo diagnóstico de brucelosis, las manifestaciones clínicas de los pacientes estudiados fueron inespecíficas, lo cual resalta la importancia del diagnóstico de laboratorio.


BACKGROUND: Human brucellosis is a global health problem. Mexico is one of the main countries affected; timely diagnosis and serological tests are the basis for detection. AIM: To know the frequency of confirmed cases of brucellosis in different of Family Medicine Units of the Mexican Social Security Institute in the state of Puebla, Mexico. METHODS: Cross-sectional study in patients of both genders, adults and pediatrics, with clinical manifestations suggestive of brucellosis; serological tests were performed for the confirmatory diagnosis. RESULTS: Out of a total of 77 patients, 39 (50.6%) were positive, 21 (27.3%) cases coming out of infection, 9 (11.7%) were negative and 8 (10.4%) were defined with immunological memory; of positive cases, 32 (82.1%) were found in the adult group and 30 (76.9%) were female. CONCLUSION: Around half of samples were confirmed as brucellosis, the clinical manifestations of the patients studied were non-specific, which highlights the importance of laboratory diagnosis.

BACKGROUND: Brucella canis is the etiological agent of canine brucellosis, a worldwide neglected zoonosis that constitutes one of the major infectious causes of infertility and reproductive failure in dogs. Although genomic information available for this pathogen has increased in recent years, here we report the first genome sequencing of a B. canis strain in Chile, and the differences in virulence genes with other B. canis strains. RESULTS: Genome assembly produced a total length of 3,289,216 bp, N50 of 95,163 and GC% of 57.27, organized in 54 contigs in chromosome I, and 21 contigs in chromosome II. The genome annotation identified a total of 1981 CDS, 3 rRNA and 36 tRNA in chromosome I, and 1113 CDS and 10 tRNA in chromosome II. There is little variation between the different strains and the SCL isolate. Phylogenetic analysis showed that the Chilean SCL strain is closely related to B. canis and B. suis strains. Small differences were found when compared to the Serbian isolate, but all strains shared the same recent common ancestor. Finally, changes in the sequence of some virulence factors showed that the SCL strain is similar to other South American B. canis strains. CONCLUSIONS: This work sequenced and characterized the complete genome of B. canis strain SCL, evidencing the complete presence of all the genes of the virB operon, and minor changes in outer membrane proteins and in the urease operon. Our data suggest that B. canis was introduced from North America and then spread throughout the South American continent.

Abstract INTRODUCTION: Leptospirosis and brucellosis cause immunosuppression that worsens the clinical condition of people living with HIV/AIDS (PLWHA). We investigated the serological profile and risk factors of PLWHA. METHODS: Serum samples (n=238) were researched for Brucella spp. antibodies using Rose Bengal and tube agglutination tests and Leptospira spp. antibodies using the microscopic agglutination test. RESULTS: All samples were negative for Brucella spp. For leptospirosis, four samples (1.69%) were positive, and Andamana was the prevalent serovar. CONCLUSIONS: Low or no detection of these zoonoses does not reduce their importance in PLWHA. Vigilant, educational, and preventive measures should be adopted.

La brucelosis es una zoonosis que en humanos está relacionada íntimamente con la enfermedad en animales domésticos y de interés económico. Presenta 2 patrones: urbano-alimentario (consumo de leche y quesos no pasteurizados) y el rural-laboral (exposición profesional al ganado infectado). El objetivo consistió en conocer las características epidemiológicas y las técnicas que arriban al diagnóstico de Brucelosis, en un consultorio del Instituto de Medicina Tropical, del 2017 al 2019. Se realizó un estudio de tipo descriptivo y corte transverso. Se realizó el análisis de 44 pacientes con diagnóstico de Brucelosis. El 25 % de los pacientes requirieron internación. El rango de edad estuvo entre 7 a 61 años, con una media de 29 años, y una moda de 22. Las puertas de entrada fueron: por contacto con cabras positivas en el campo de práctica, faenadores, veterinarios, vacunadores, y por ingesta de leche sin pasteurizar. Las muestras serológicas por reacción de Huddleson fueron positivas a bajas diluciones; Aglutinación en tubo: 31 muestras positivas; aglutinación con 2 mercaptoetanol: 11 muestras positivas. Se llegó a la identificación de cepas Brucella melitensis biotipo 1: en 13 pacientes. Nuestro estudio indica la presencia de brucelosis humana, fundamentalmente por contacto directo debido a razones profesionales y /o laborales. La vigencia de Brucelosis deriva de una situación endémica en el ganado, y por tanto se deberán intensificar las medidas del saneamiento del ganado (vacunación) y la educación de la población de riesgo.


Brucellosis is a zoonosis in humans that is closely related to the disease in domestic and of economic interest- animals. It presents 2 main patterns: urban-alimentary (for consumption of unpasteurized milk and cheeses) and the rural-occupational (for professional exposure to infected cattle). The objective was to know the frequency of cases and the different laboratory techniques that arrives to the diagnosis of Brucellosis, in the Tropical Medicine Institute (IMT by its acronym in Spanish) during the period of 2017 to 2019. It has been made a descriptive and cross-sectional investigation. It was based on the analysis of 44 patients diagnosed with Brucellosis. 25% of the patients (11) required hospitalization. The age range was between 7 to 61 years, with a mean of 29 years old and a mode of 22. The portals of entry were: by contact with positive goats in the field of practice, slaughterhouses, veterinarians, vaccinators, and consume of unpasteurized milk. The serological samples by Huddleson reaction were positive with low dilutions; tube agglutination: 31 positive samples; agglutination with 2 mercaptoethanol: 11 positive samples. Biotype 1 of Brucella melitensis: identified in 13 patients. Our study indicates the existence of human brucellosis, mainly through direct contact due to professional and/or occupational reasons. This incidence must derive from an endemic situation in cattle, and therefore the measures of sanitation of cattle (vaccination) and education of the population at risk should be intensified

ABSTRACT

The objectives of this study were to estimate: (a) the frequency of zoonoses in large animal veterinarians from rural areas of the province of Buenos Aires, Argentina, and (b) to describe the use and disposal of personal protective equipment (PPE) and selective veterinary clinical waste. A cross-sectional study was carried out on large animal veterinary practitioners in the Province of Buenos Aires (n = 106). One third (29.2%) of them had been diagnosed with a zoonosis by laboratory-methods, being brucellosis the most frequent (22.6%). The more years passed since their graduation, the greater the chances of becoming ill (p < 0.001). Gloves were the most adopted PPE; however, other elements had little or no use at all. Older and experienced professionals used PPE less frequently than young inexperienced practitioners. Some PPE was frequently reused and the final disposal of veterinary waste was often inappropriate. A change in behavior is an urgent need to preserve not only the veterinarians' health but also their families' wellbeing and to ensure proper disposal of potentially hazardous waste.


Los objetivos de este trabajo fueron los siguientes: a) estimar la frecuencia de zoonosis en veterinarios de grandes animales que desarrollan su labor en la provincia de Buenos Aires; b) describir el uso y la disposición de los elementos de protección personal (EPP) y otros residuos generados durante el trabajo de estos veterinarios. Para ello se realizó un estudio transversal en una muestra de 106 profesionales. En un tercio de ellos (29,2%) se había sido diagnosticado por métodos de laboratorio alguna zoonosis; la brucelosis fue la más frecuente (22,6%). Se encontró que a mayor tiempo transcurrido desde la graduación, mayor era la probabilidad de enfermarse (p< 0,001). Sobre la base de las respuestas obtenidas en cuestionarios estructurados, se determinó lo siguiente: que los guantes fueron el EPP más adoptado, mientras que otros elementos tuvieron escasa o nula adopción; que los profesionales de mayor edad y experiencia usaron EPP con menos frecuencia que los practicantes más jóvenes e inexpertos; y que algunos EPP se reutilizaban con frecuencia y que la eliminación final de los desechos veterinarios fue a menudo inapropiada. Considerando estos hallazgos, es claro que se requiere un cambio de comportamiento para preservar no solo la salud de los veterinarios, sino también para asegurar el bienestar de sus familias y garantizar la eliminación adecuada de los residuos potencialmente peligrosos.

Introducción: La Brucelosis es una zoonosis común y en muchos países es un problema sanitario prevalente. En Paraguay no se ha encontrado evidencia de los conocimientos que tienen los trabajadores de hatos lecheros y las prácticas preventivas que utilizan. Objetivo: Evaluar el conocimiento, las prácticas de prevención y de bioseguridad sobre Brucelosis de los trabajadores expuestos a riesgo en hatos lecheros del departamento de Caaguazú, Paraguay. Materiales y métodos: Se realizó un estudio transversal, en una muestra representativa de establecimientos ganaderos del Departamento de Caaguazú. De mayo a junio de 2017 se entrevistó a 720 trabajadores de 360 hatos lecheros, utilizando un cuestionario estructurado, con preguntas cerradas. Se obtuvo información de los conocimientos, prácticas de prevención y bioseguridad sobre Brucelosis. Resultados: La mediana de edad de los trabajadores fue 34 años, 70% eran hombres. El 11,7% era analfabeto y 62,1% solo culminó la primaria. El 90% se desempeñaba como tambero/ordeñador. El 64% manifestó que no conocía la enfermedad, 73,6% ignoraba la forma en que se adquiere la enfermedad, 71,5% nunca recibió información acerca de la Brucelosis. El 73,3% no tenía conocimientos de las prácticas preventivas. Todos consumían subproductos lácteos sin pasteurizar, principalmente queso paraguay Conclusión: Existe desconocimiento de la enfermedad y la forma de transmisión. Las prácticas de riesgo son frecuentes. Es necesario implementar programas de educación y concienciación sobre la Brucelosis. Background: Brucellosis is a common zoonosis and in many countries it is a prevalent health problem. In Paraguay, no evidence has been found of the knowledge that dairy herd workers have and the preventive practices they use. Objective: To evaluate the knowledge, prevention and biosecurity practices on Brucellosis of workers exposed to risk in dairy herds of Caaguazú. Materials and methods: A cross-sectional study was carried out in a representative sample of livestock establishments in the Department of Caaguazú. From May to June 2017, 720 workers from 360 dairy farms were interviewed, using a structured questionnaire with closed questions. Information on the knowledge, prevention and biosecurity practices on brucellosis was obtained. Result: The median age of the workers was 34 years, 70% were men. 11.7% were illiterate and 62.1% only completed the primary. 90% worked as a tambero/milker. Sixty-four percent said they did not know about the disease, 73.6% did not know how the disease was acquired, 71.5% never received information about brucellosis. 73.3% had no knowledge of preventive practices. All consumed unpasteurized dairy byproducts, mainly Paraguayan cheese. Conclusion: There is a lack of knowledge of the disease and the way of transmission. Risk practices are frequent. It is necessary to implement education and awareness programs on brucellosis. Keywords: Brucellosis; knowledge; prevention practices; biosecurity;survey; human.

Abstract Background and aim Brucellosis is an infectious, contagious and zoonotic disease that occurs worldwide. The family members of an index case of brucellosis may be especially susceptible, due to sharing the same source of infection and similar risk factors for brucellosis. In this study, we propose to screen pediatric and adult family members of brucellosis index cases for detecting additional unrecognized infected family members. Materials and methods 114 family members of 41 pediatric patients with brucellosis were evaluated. All family members completed a brief questionnaire and were tested by a standard tube agglutination test (STA). Results The majority of family members (n = 96, 84.2%) were children. Among the 114 family members, 42 (36.8%) were seropositive, and 15 (35.7%) were symptomatic. The majority of the symptomatic seropositive family members (n = 12, 80%) had STA titers (≥1:640) higher than asymptomatic seropositive family members (n = 9, 33%; p = 0.004). Conclusion The routine screening of both pediatric and adult family members of index cases is a priority in endemic areas. Using this screening approach, unrecognized family members who are seropositive for brucellosis will be identified earlier and be able to receive prompt treatment.

Introduction: Brucellosis is a zoonosis caused by Brucella spp. It may be acquired by consuming unpasteurized dairy products. Brucellosis has a low incidence in Chile, thus, we have a scarce data. Aim: To report and to characterize the first series of clinical cases of adult patients diagnosed with brucellosis in Chile. Methods: We describe a series of 13 clinical cases in patients diagnosed between 2000 and 2016 in three different centers in the Metropolitan Region, Chile. A retrospective analysis was performed on clinical presentation, laboratory, antibiotic treatment, morbidity and mortality. Results: The mean age was 50 years old. Eight cases had a record of consumption of unpasteurized dairy products. The most frequently reported complaints were fever. The most frequent focal point involved was the spine. Only one patient had a positive blood culture, while the diagnosis was made using serological techniques in the other part of the group. The most indicated antibiotic regimens were doxycycline-rifampicin and doxycycline-gentamicin. The hospital stay was 20 days approximately as an average. Clinical cure was achieved in all cases. Conclusions: Brucellosis is an infrequent zoonosis in Chile, and it produces a nonspecific clinical picture, so it is necessary to have high suspicion to make the diagnosis based in the antecedent of consumption of unpasteurized dairy or raw meat.


Introducción: La brucelosis es una zoonosis provocada por Brucella spp, cuyo principal mecanismo de transmisión es mediante el consumo de lácteos no pasteurizados. Es de baja ocurrencia en Chile, por lo que los datos locales son escasos. Objetivo: Reportar y caracterizar la primera serie de casos clínicos de pacientes adultos con diagnóstico de brucelosis en Chile. Material y Método: Se describe una serie de 13 casos clínicos en pacientes diagnosticados entre el año 2000 y el 2016 en tres centros de la Región Metropolitana, Chile. Se realizó un análisis retrospectivo acerca de la presentación clínica, laboratorio, tratamiento antimicrobiano y morbi-mortalidad. Resultados: El promedio de edad fue 50 años. Ocho casos tenían antecedente de consumo de lácteos no pasteurizados. El motivo de consulta más frecuente fue sensación febril. El compromiso focal más frecuente fue el de columna vertebral. Sólo en un paciente se encontró hemocultivo positivo; en el resto el diagnóstico se hizo mediante técnicas serológicas. Los esquemas antimicrobianos más indicados fueron doxiciclina-rifampicina y doxiciclina-gentamicina. La estadía hospitalaria fue en promedio de 20 días. En todos los casos se logró curación clínica. Conclusiones: La brucelosis es una zoonosis infrecuente en Chile, produce un cuadro clínico inespecífico, por lo que se debe tener una alta sospecha para realizar el diagnóstico, basada en el antecedente del consumo de productos lácteos no pasteurizados o de carne mal cocida.

ABSTRACT OBJECTIVE The objective of this study is to estimate the seroprevalence of human brucellosis in livestock professionals and analyze the factors associated with brucellosis focusing on sociodemographic variables and the variables of knowledge and practices related to the characteristics of the activities carried out in livestock. METHODS This is a cross-sectional seroepidemiological study with a population of 131 workers of butchers, slaughter rooms, and slaughterhouse and 192 breeders sampled randomly in Namibe province, Angola. The data were obtained from the collection of blood and use of questionnaires. The laboratory tests used were rose bengal and slow agglutination. The questionnaire allowed us to collect sociodemographic information and, specifically on brucellosis, it incorporated questions about knowledge, attitudes, and behaviors of livestock professionals. In addition to the descriptive statistical approach, we used the Chi-square test of independence, Fisher’s test, and logistic regression models, using a significance level of 10%. RESULTS The general weighted prevalence of brucellosis was 15.6% (95%CI 13.61–17.50), being it 5.3% in workers and 16.7% (95%CI 11.39–21.93) in breeders. The statistical significance was observed between human seroprevalence and category (worker and breeder) (p < 0.001) and education level (p = 0.032), start of activity (p = 0079), and service location (p = 0.055). In a multivariate context, the positive factor associated with brucellosis in professionals was the professional category (OR = 3.54; 95%CI 1.57–8.30, related to breeders in relation to workers). CONCLUSIONS Human brucellosis in livestock professionals is prevalent in Namibe province (15.6%), where the professional category was the most important factor. The seroprevalence levels detected are high when compared with those found in similar studies.


RESUMO OBJECTIVO Estimar a seroprevalência da brucelose humana em profissionais da pecuária e analisar os factores associados à brucelose com foco em variáveis sociodemográficas, de conhecimento e práticas relativas às características das actividades desenvolvidas na pecuária. MÉTODOS Estudo transversal seroepidemiológico em população de 131 trabalhadores de talhos, salas de abate e matadouro e 192 criadores amostrados aleatoriamente na província do Namibe, Angola. Os dados foram obtidos por meio de colheita de sangue e aplicação de questionário. Os testes laboratoriais utilizados foram o Rosa de Bengala e a aglutinação lenta em tubos. O questionário permitiu recolher informação sociodemográfica e, especificamente sobre a brucelose, incorporou questões sobre conhecimento, atitudes e comportamentos dos profissionais da pecuária. Além da abordagem estatística descritiva, foram utilizados os testes de Independência do Qui-quadrado, Fisher e modelos de regressão logística, utilizando um nível de significância de 10%. RESULTADOS A prevalência geral ponderada da brucelose foi de 15.6% (IC95% 13.61–17.50), sendo 5.3% em trabalhadores e 16.7% (IC95% 11.39–21.93) em criadores. A significância estatística foi observada entre a seroprevalência humana e a categoria (trabalhador e criador) (p < 0.001) e o nível de instrução (p = 0.032), início de actividade (p = 0.079) e local de serviço (p = 0.055). Em um contexto multivariado, o factor positivamente associado à brucelose em profissionais foi a categoria profissional (OR = 3.54; IC95% 1.57–8.30, relativo aos criadores em relação a trabalhadores). CONCLUSÕES A brucelose humana em profissionais da pecuária é prevalente na província do Namibe (15.6%), onde a categoria profissional foi o factor mais relevante. Os níveis de seroprevalência detectados são elevados se comparados com outros encontrados em estudos semelhantes.

ABSTRACT

Introduction: Brucellosis is one of the most frequent zoonosis in most parts of the world. This zoonosis remains a great problem to public health in developing countries, although developed countries have successfully controlled it. Mexico still shows a high annual brucellosis incidence in humans; thus, the country is considered around the world as an endemic brucellosis country. Aim: To describe the connection/association between this zoonosis and the current epidemiological situation in the Mexican population. Methods: Perusal of research reports, epidemiological studies and veterinarian reviews performed in Mexico, using data bases such as PubMed, Thompson Reuters, Mesh research. Conclusion: The risk of infection by Brucella in Mexico is associated with the consumption of unpasteurized dairy products, mainly fresh cheeses.


Introducción: La brucelosis es una de las enfermedades zoonóticas más frecuentes en la mayor parte del mundo. Mientras que en los países desarrollados han logrado con éxito su control, en los países en vías de desarrollo continúa siendo un gran problema de salud pública. México continúa presentando alta incidencia anual de brucelosis en humanos, por lo que es considerado un país endémico de brucelosis. Objetivo: Describir la relación de esta zoonosis con la situación epidemiológica actual en la población de México. Material y Métodos: Consulta de reportes de investigación, estudios epidemiológicos y revisiones veterinarias, realizadas en México, a través de bases de datos como PubMed, Thompson Reuters y Meshresearch. Conclusión: El riesgo del contagio de Brucella spp. en México está asociado al consumo de productos lácteos sin pasteurizar, principalmente quesos frescos.

Abstract: Objective: To describe the seroprevalence and associated factors for brucellosis among dairy farm workers. Materials and methods: We performed a secondary analysis of a data set and sera from a previous cross-sectional study in a dairy farm. Sera were tested for Brucella spp. antibodies by the slide agglutination test. Seropositivity was defined as a titer ≥1:40; recent infection was titers ≥1:160. Results: We tested 331 human sera. Seroprevalence of brucellosis was 18.1% (60/331; 95% CI 14.1-22.7); 13.3% of them (8/60; 95% CI 5.9 24.5) corresponded to recent infection. Highexposure occupation (calf caretaker; OR 3.3; 95%CI 1.1 - 9.7), daily hours in contact with cows (OR 1.1; 95%CI 1.03 - 1.2), and living on-site (OR 2.2; 95% CI 1.1 - 4.4) remained independently associated with seropositivity. Conclusions: We found a high seroprevalence of brucellosis among dairy farm workers, as well as a significant association among those with prolonged and close contact with cattle.


Resumen: Objetivos: Describir la seroprevalencia y factores asociados con la brucelosis en los trabajadores de una cuenca lechera. Material y métodos: Se realizó un análisis secundario de datos y sueros obtenidos en una cuenca lechera. Se buscaron anticuerpos contra Brucella spp. en los sueros por medio de la prueba de aglutinación en placa. Se definió seropositividad a partir de un título ≥1:40, e infección reciente con títulos ≥1:160. Resultados: Se analizaron 331 sueros humanos. La seroprevalencia de brucelosis fue de 18.1% (60/331; IC 95% 14.1-22.7); el 13.3% (8/60; IC 95% 5.9 24.5) correspondieron a infección reciente. Alta exposición (becerrero; RM 3.3; IC 95% 1.1 - 9.7), horas diarias en contacto con vacas (RM 1.1; IC 95% 1.03 - 1.2), y vivir en el establo (RM 2.2; IC 95% 1.1 - 4.4) estuvieron asociadas independientemente con seropositividad. Conclusiones: Se encontró alta seroprevalencia de brucelosis en trabajadores de una cuenca lechera, y asociación en aquellos con contacto cercano y prolongado con vacas.

ABSTRACT

Se realizó un estudio epidemiológico de brucelosis en 516 majadas caprinas o mixtas (caprinos/ovinos) de las 3 regiones agroecológicas de la provincia de Formosa, Argentina. Mediante las pruebas de aglutinación en placa con antígeno tamponado y de fijación del complemento en suero se estudiaron un total de 25.401 caprinos y 2.453 ovinos. Además, se realizaron cultivos bacteriológicos y PCR en muestras de leche de cabras de 3 majadas con brucelosis y abortos recientes. Se detectó brucelosis en 4 de los 9 departamentos de la provincia, la prevalencia global fue del 2 % y la intrapredial varió entre el 1 y el 40%. La proporción de majadas positivas fue del 3,6, el 12 y el 36 % para las regiones este, centro y oeste, respectivamente. Se aisló Brucella melitensis bv. 1 de cabras por primera vez en la provincia. La PCR amplificó fragmentos esperados de 827 pb correspondiente al gen omp2ab (Brucella spp.) y de 731 pb correspondiente al inserto IS711 (B. melitensis). La detección de anticuerpos en ovinos que cohabitan con caprinos sugiere que las infecciones habrían sido causadas por B. melitensis, lo que constituye un riesgo adicional para la salud pública. Los programas de control y erradicación de la brucelosis deberían considerar las majadas mixtas como una sola unidad epidemiológica. Los resultados indican que la brucelosis por B. melitensis bv. 1 es altamente endémica en las regiones centro y oeste de la provincia de Formosa.


An epidemiological study of brucellosis was carried out in 516 goats and mixed flocks (goat/sheep) from the three agro-ecological regions of Formosa province, Argentina. Serum samples from a total of 25401 goats and 2453 sheeps were analyzed using buffered plate agglutination test (BPAT) and complement fixation test (CFT). Bacteriological and PCR analyses on milk samples from goats in three flocks with a history of brucellosis and recent abortions were also performed. Brucellosis was detected in four of the nine departments of the province with an overall prevalence of 2 % and an intra-flock prevalence ranging between 1 % and 40 %. The proportion of infected flocks was 3.6 %, 12 % and 36 % for the eastern, central and western regions, respectively. Brucella melitensis bv. 1 was isolated efrom goats for the first time in the province. The expected fragments of 827 bp from the omp2ab gene (Brucella spp.) and 731 bp from the insert IS711 (B. melitensis) were amplified by PCR. Detection of antibodies by BPAT and FCT in sheep cohabiting with goats suggests that infections could have been caused by B. melitensis, posing an additional risk to public health. Control and eradication programs for brucellosis should consider mixed flocks as a single epidemiological unit. The results indicate that brucellosis by B. melitensis bv1 is highly endemic in the central and western regions of Formosa province.

Objetivo. Determinar el comportamiento de la incidencia de brucelosis humana en México durante el periodo 2000-2011 y su relación con la brucelosis en rumiantes domésticos. Material y métodos. En estudio ecológico de series de tiempo se analizaron, mediante regresión Poisson multinivel múltiple, la incidencia de brucelosis humanas durante 2000-2011 y la incidencia en rumiantes, y su relación con índice de desarrollo humano. Resultados. La alta incidencia de brucelosis bovina aumenta 15% la incidencia de brucelosis humana. La alta incidencia en caprinos aumenta 33% la incidencia de brucelosis humana. La presencia de casos nuevos de brucelosis ovina aumenta 13% la incidencia de brucelosis humana. Conclusiones. La brucelosis es un problema de salud pública; la presencia de la brucelosis en los rumiantes domésticos y el nivel de control de la enfermedad en ellos afecta la presencia de la enfermedad en humanos.


Objective. To determinate the relationship of human brucellosis incidence in Mexico during 2000-2011 in relation with domestic ruminant brucellosis. Materials and methods. An ecological time series study was analyzed using multilevel poisson multiple regression, the incidence of human brucellosis during 2000-2011 and incidence in ruminants and human development index. Results. The high incidence of bovine brucellosis increased 19% the incidence of human brucellosis. The high incidence of caprine brucellosis increase 17% the incidence of human brucellosis. The presence of new cases of ovine brucellosis increased 13% the incidence rates of human brucellosis. Conclusions. Brucellosis is a public health problem in Mexico, the presence of brucellosis in domestic ruminants and the level of disease control in them affects the presence of the disease in humans.

Objective: to determine the seroprevalence of Brucella spp in humans.Method: this is an observational study, developed with 455 individuals between 18 and 64 years old, who use the Estratégia de Saúde da Família (Brazil's family health strategy). The serum samples of volunteers underwent buffered acid antigen tests, such as screening, agar gel immunodiffusion and slow seroagglutination test in tubes and 2-Mercaptoethanol.Results: among the samples, 1.98% has responded to buffered-acid antigen, 2.85% to agar gel immunodiffusion test and 1.54% to the slow seroagglutination tests on tubes/2-Mercaptoethanol. The prevalence of Brucella spp was 4.4%, represented by the last two tests.Conclusion: the results of this research suggest that the studied population is exposed to Brucella spp infection.


Objetivo: determinar a soroprevalência da Brucella spp em humanos.Método: trata-se de estudo observacional, desenvolvido com 455 indivíduos entre 18 e 64 anos, selecionados, que utilizavam a estratégia de saúde da família. As amostras de soro dos voluntários foram submetidas aos testes de antígeno acidificado tamponado, como triagem, imunodifusão em gel de ágar e aos testes de soroaglutinação lenta em tubos e 2-mercaptoetanol.Resultados: dentre as amostras, 1,98% reagiram ao antígeno acidificado tamponado, 2,85% à imunodifusão em gel ágar e 1,54% aos testes de soroaglutinação lenta em tubos/2-mercaptoetanol. Sendo a prevalência da Brucella spp de 4,4%, representada pelos dois últimos testes.Conclusão: os resultados desta pesquisa sugerem que a população estudada encontra-se exposta à infecção por Brucella spp.

La prevalencia de laBrucella spp representada por las dos últimas pruebas fue del 4,4%.Conclusión: los resultados de esta investigación sugieren que la población que ha sido estudiada se encuentra expuesta a la infección por Brucellaspp.

Brucellosis and leptospirosis are widely spread bacterial infections and dogs are the most important source of infection and reservoir for diseases. Dogs can disseminate the agents in the environment and transmit them to humans and/or other animals. The objective of this study was assess the occurrence of reactive to antibodies anti-Leptospira spp., Brucella canis and B. abortus in Belém and Castanhal, State of Pará, Amazon, Brazil. A total of 156 samples were randomly collected in the city of Belém and 158 samples in Castanhal. The anti-B. canis antibodies research was performed by Agar Gel Immunodiffusion (AGID) with and without 2-mercaptoethanol serum treatment (AGID-2ME). To assess the anti-B. abortus antibodies, the technique of Fast Seroagglutination with buffered acidified plate antigen (BAPAT) was used. For anti-Leptospira spp. antibodies research, the Microscopic Agglutination Technique (MAT) was used. No animal reacted to Brucella abortus and one animal was reactive to B. canis at the AGID, but it was negative to the AGID-2ME test. Seventeen percent of dogs (47/274) presented anti-Leptospira spp. antibodies, with prevalence of serovar Canicola. The dogs from Belém and Castanhal are not source of infection for B. abortus and B. canis, however, they are reservoirs for different serovars of Leptospira spp.


Brucelose e leptospirose são enfermidades bacterianas amplamente disseminadas e o cão é importante fonte de infecção e reservatório para ambas, podendo eliminar o agente no meio ambiente, e transmiti-lo para humanos e/ou outros animais. O presente estudo teve como objetivo pesquisar a ocorrência de cães reagentes para anticorpos contra Leptospira spp., Brucella canis e B. abortus em Belém e Castanhal, Pará, Amazônia, Brasil. Foram colhidas de forma aleatória 156 amostras no município de Belém e 158 amostras em Castanhal. A pesquisa de anticorpos anti-B. canis foi realizada pela técnica de Imunodifusão em Gel de Ágar (IDGA) com e sem tratamento do soro com 2-mercaptoetanol (IDGA-2ME) e para pesquisa de anticorpos anti-B. abortus foi utilizada a técnica de Soroaglutinação Rápida com antígeno acidificado tamponado (AAT). Para pesquisa de anticorpos contra-Leptospira spp. utilizou-se a Técnica de Aglutinação Microscópica (MAT). Nenhum animal reagiu para Brucella abortus e um animal foi reagente para B. canis na IDGA, porém foi negativo na IDGA-2ME. Dezessete por cento dos cães (47/274) apresentaram anticorpos contra-Leptospira spp., com predominância do sorovar Canicola. Os cães de Belém e Castanhal não são fontes de infecção para B. abortus e B. canis, no entanto são reservatórios de diferentes sorovares de Leptospira spp.

Background: Brucellosis is a major zoonotic disease that is endemic in Saudi Arabia and it remains a major health problem that has not been eradicated in the country yet. Place and Duration of Study: This retrospective study was conducted in a Saudi Hospital at Al Madinah city during the period of 1 November, 2010 to 31 October, 2011. Methodology: All sera of patients suspected to have brucellosis (n= 65) and 18 healthy subjects were tested for brucella antibody using slide latex agglutination (SAT) and ELISA. Quantitation of IFN-ɣ was also done using ELISA. Results: Brucellosis was detected in all age groups but the incidence was higher and reached 33.3% in age group (40- <50) years with average of 43.9±2.53 years. Male to female ratio in infected patients was 2:1 by using SAT. The incidence of seropositive cases was high (80.1%) in the three months (April, May and June), with the highest peak in May (46.7%). Drinking raw milk was the most encountered risk factor with a prevalence of 66.1% followed by consumption of milk products (11.9%). The most prevalent species among the examined cases was B. melitensis (93.3%). Among the studied cases, 60 cases (92.3%) were serologically positive for brucellosis by SAT. Among the 60 cases yielding significant titers against brucella, 14 sera (23.3%) had agglutinin levels of 1:80, 34 sera (56.7%) had titers of 1:160 and 12 sera (20%) had titers of 1:320. By estimating IgM and IgG levels in the sera of examined cases using ELISA, 52 cases (80%) had brucellaIgM while 42 cases (64.6%) had brucella IgG. Sensitivities of SAT, IgM ELISA and IgG ELISA were 91.5%, 88.1% and 71.2%, respectively compared with combined ELISA. Mean IFN-ɣ levels ± SD in the subacute phase was 136.7±70.07pg/ml, 120.2±54.25pg/ml in the acute phase, and 121.3±51.09 pg/ml in the chronic phase of brucellosis. Conclusion: The sensitivity and specificity of ELISA to diagnose human brucellosis was higher when combined ELISA (IgM/IgG or both) was used. Mean IFN-ɣ levels were lower, but not significantly, in the chronic phase of the disease than in the sub acute phase and healthy subjects.

In the present study, the outbreak patterns of bovine brucellosis in Korea from 2000 to 2011 were analyzed to understand the epidemiological evolution of this disease in the country. A total of 85,521 brucella reactor animals were identified during 14,215 outbreaks over the 12-year study period. The number of bovine brucellosis cases increased after 2003 and peaked in 2006 before decreasing thereafter. The majority of the bovine brucellosis cases were Korean native cattle, Han Woo. The numbers of human brucellosis cases and cattle outbreaks increased and decreased in the same pattern. The correlation coefficient for human and bovine cases per year was 0.96 (95% confidence interval = 0.86~0.99; p < 10(-3)). The epidemiological characteristics of bovine brucellosis appeared to be affected by the intensity of eradication programs that mainly involved a test-and-slaughter policy. Findings from the present study were based on freely available statistics from web pages maintained by government agencies. This unlimited access to information demonstrates the usefulness of government statistics for continually monitoring the health of animal populations.
